GENERAL QUESTIONS:

  • The best way to get our forms would be to either come to the Youth Alliance Administrative Office, located at 310 4th St. Suite 101 in Hollister. After School Applications can be picked up at the school sites as well.
  • All of Our programs are provided free of cost!

  • See the CAMINOS page for more detailed information. But, you need to fill out a Mental Health Screening Tool, which can be downloaded here:

    MHST Youth Ages 6 to 17 MHST Children Ages 0 to 5

    Then, you can either bring it to the Youth Alliance Main Office, Fax the form to (831) 636-2850, or contact your schools’ Principal.

  • CAMINOS School Based Counseling typically focuses on shorter term goals, whereas SBC Behavioral Health works with children and teens who need longer-term support. Additionally, YA services do not require Medi-Cal or Insurance eligibility.
